Travel assistance request

Apply online for travel assistance for a child or young person.

Please complete this form if you are making a request for travel assistance for a child or young person based on one of the following:

  • eligibility;
  • the distance measurement in relation to statutory walking distances;
  • the safety of the walking route;
  • the walking route is unsuitable based on the child's specific needs;
  • alterations are needed to the transport arrangements already provided;
  • exceptional circumstances, not listed above, which are temporary or permanent in nature.

Please ensure that you read all questions carefully. You must submit relevant evidence, in the form of additional documents, in support of your request. Please note a failure to provide appropriate evidence may result in your case being refused, any costs with providing evidence will need to be funded by you as the applicant.

A senior officer will review your case and make a decision based on the evidence you provide, within 20 working days. If travel assistance is granted, Swansea Council will determine how this assistance will be provided and any review date. We are unable to guarantee that individual transport or specific drivers and passenger assistants will be provided.

If travel assistance is granted, any arrangements will take a minimum of 10 working days to put in place from the date transport is agreed / awarded.
