I can confirm that I have spoken to the parent / carer in relation to the possibility of ALN, and the parent / carer agrees for the Early Years ALN Support and Inclusion Team to be involved with their child, if required. The parent / carer understands the purpose of this is to help understand and explore how to support their child's development and wellbeing.
2. The parent / carer understands and recognises that in order to provide the ALN support required for their child, the ALN and Inclusion Team will need to do the following:
  1. • Information about their child will be shared with Education Directorate.
    
    • The Education Directorate may gather information from any other professional involved with their child.
    
    • Their child's details will be stored securely by the ALN and Inclusion Team.
    
    • Information arising from involvement with the ALN and Inclusion Team will be shared with other professionals.
