Special procedure licence application form (Person)

Before you complete the special procedures form

You will need the following information to complete this form. You should make sure you have all of this before you start filling in the form.

1. A Basic Disclosure Certificate which is dated within the last three months OR an overseas criminal record certificate.

You can apply for a basic DBS check on the Gov.uk website (opens new window). The application form is online and you will need to create an account if you don't already have one. You will also need to prove your identity as part of the application and may need to upload ID. The cost of a basic DBS check is £21.50.

An applicant who is not eligible for a basic disclosure certificate must obtain an overseas criminal record certificate. The application process for criminal record checks for an applicant from overseas varies from country to country. An applicant may need to apply in the country or to the relevant embassy in the UK. 2. A copy of your public liability insurance cover.

3. A copy of your Level 2 Award in Infection Prevention and Control for Special Procedures Practitioners. You can find more information on how to gain this qualification at Guidance note 15.

4. A copy of your identity documents. You can find more information on what identity documents are needed at Guidance note 16.

5. A recent colour photograph of the applicant. You can find more information about this at Guidance note 17.

6. A payment method to pay your fee online. We can accept debit and credit card (Visa, Mastercard, Maestro, Electron) payments through this form. We are unable to accept cash payments
