Individual Development Plan referral

An Individual Development Plan (IDP) is a plan created and agreed by those people most closely involved with supporting a child or young person, including parents / carers.

If a learner is in an education setting, they are best placed to devise an IDP. This is a collaborative process, which will place the needs and aspirations of the learner at the heart of it.

If you are a parent / carer and you wish to request an IDP, please discuss this with your child's education setting in the first instance. If you are in a disagreement with the education setting, please contact the casework team before completing this form, as they will be able to support you, email: caseworker@swansea.gov.uk

1. We need to know the type of referral you are completing.
  1. 1. A referral for a local authority maintained Individual Development Plan for a learner who attends school, further education or with no current provision.
    
    2. A referral for a local authority maintained Individual Development Plan for a child in an early years setting and / or a request for equipment and / or training for early years settings not linked directly to an individual child.
    
    3. A referral for the local authority to consider placement for an END OF KEY STAGE learner to move to an STF or Specialist Provision, this should be made in Y5 or Y10 (summer term).
    
    4. A referral for a child who currently has a specialist placement with a local authority IDP which is no longer required.
