Gambling licences application forms
Application forms are available for premises licences, permits, provisional statements, lotteries and notices.

When you submit your application form you should also send copies to each of the responsible authorities. Each of these responsible authorities is able to make a representations to us about the applications we receive under the Gambling Act 2005. We take these comments into account when issuing a licence.
A temporary use notice can allow the use of premises for gambling without the need for a premises licence.
The occasional use notice allows betting at racetracks for short periods, without the need for a betting premises licence.
If you are making an application for the grant or variation of a premises licence or for a provisional statement, notice must be given to each of the responsible authorities. A notice should also be displayed at the premises where members of the public are able to view it. The notice should also be published in a local newspaper. These notices should use the following templates.
