Government funded childcare offer - continued eligibility declaration

As part of our ongoing checking process, parents will be asked to complete a declaration form each term. This will re-affirm their continued eligibility going forward into the next term.

There will also be a random selection of applications required to re-submit up to date payslips.

To ensure continued funding for the childcare offer, you are required to declare your continued eligibility for the spring term 2022 by answering the questions below and submitting your responses.

Failure to return the declaration may result in your funded childcare being stopped until proof of eligibility is confirmed. From the data you provide us with, we may contact you by email and / or telephone to gather further information.
