GP death reporting
The Coroner's Service for Swansea and Neath Port Talbot are currently rolling out a new electronic system for any referrals to the HM Coroner.
This will involve entering the information directly on to the Coroner system via a portal. This will ensure that the information to the Coroner is accurate as received directly from the practice.
The new system works by accessing a link to our portal where the data is input directly.

The link that your team must use to access the referral page on our portal is https://coronersswanseaportal.icasework.com/form?Type=CorornersReferral&Login=False.
The same link can be used by everyone as it isn't user specific. This can be saved as a 'favourite' in their browser and it best works with either Microsoft Edge or Google Chrome.

On page 2 the 'Referrer code' will need to be entered, which for GPs is GP01 (third character is a zero). Also at the top of Page 2 is the option to upload any supporting documents such as the MCCD or Patient Summary etc. The Coroner would prefer the Patient Summary to be uploaded with the referral if this is possible.
On Page 4 please enter the reason for referral from the drop-down menu and more information can be put in the 'Detailed reasoning' box.
