WESTERN BAY CHANGE COORDINATOR – COMMISSIONING (X3)

This job is only available to staff of City and County of Swansea, Bridgend County Borough Council, Neath Port Talbot County Borough Council and the Abertawe Bro Morgannwg University (ABMU) Health Board

Fixed term contracts — 6 months, until 31st March 2015.

You will play a key role in supporting change across the Western Bay region in relation to the commissioning of care packages. You will assist Team Leaders across Adult Services in the development and implementation of right sizing of existing care packages based on the Pathways to Independence (PTI) methodology across adult services.

Additionally you will support the implementation of a robust assessment and right sizing process which works alongside existing assessment processes so that packages are appropriately delivered and reflected in the support plans

You will need significant and relevant post professional qualifying experience in social work, have experience of negotiating and change management and be able to manage stakeholder expectations.

Knowledge of the assessment process, demonstrating a commitment to the production of maximising independence is essential as is a detailed knowledge of assessment for at risk adults. You will also need a good understanding of Information Technology and a commitment to developing their use in Social Work.

You will play a key role in supporting service improvement work and facilitating change as part of the Western Bay Health and Social Care Collaborative’s Mental Health Project. Duties will include:

 Providing expertise, advice and practical support for delivering improved services for individuals
 Communicating and championing good practice and service improvement across the Mental Health services provided throughout the Western Bay Region.
 Supporting the adoption and spread of service improvement approaches and methodologies
 Working with stakeholders to make sustainable changes and improvement, through advice, teaching and coaching in service redesign and change management.

Working with Practice Solutions Ltd to support the delivery of an agreed programme of change across the Western Bay area.

Educated to degree level and possessing a health or social care qualification, you must possess strong project manager skills and have a good understanding and knowledge of the impact that mental health and wellbeing can have upon individuals and their families.

Experience of mental health service delivery, change management and of working in a social care, health or equivalent relevant setting is essential.

TEMPORARY PART TIME FLYING START CHILDCARE WORKER (18 HOURS PER WEEK, 42 WEEKS)

Flying Start provision at Sea View offers free childcare for children 2-3 years living in designated areas. The provision of good quality childcare for children 2-3 years of age is the centrepiece of services to be delivered under the Flying Start initiative. The quality childcare provision will focus on children from disadvantaged backgrounds, in target areas, and aims to improve their outcomes in preparation for school and in the long term
The successful applicants will be at least 18 years of age and have a relevant childcare qualification at a minimum of level 3 and experience of providing appropriate play provision for under 3’s. Whilst the main base will be at Sea View you must be willing to work flexibly at other sites as necessary.
The Childcare worker post includes working as part of a strong team to provide excellent quality child care for all children attending the facility.
We are looking to fill 2 part time posts and these posts are expected to be for 18 hours per week, 42 weeks a year.
These posts are funded by the Welsh Government and currently has funding until March 31st 2015 with a view to be annually extended

A vacancy has arisen for a temporary part time Business Support Assistant in our Adoption Team currently based at Cockett House.

You will be required to provide administrative, typing and reception support for the Adoption Team.

You would need to be flexible with excellent IT skills and have the ability to work under pressure and manage your time in what is a very busy and challenging environment.

Candidates need to have 3GCSE`s or equivalent including English and Maths. RSA stage 2 Word Processing or equivalent would be an advantage.

The post is 18.5 hours per week which are worked over Wednesday pm, all day Thursday and all day Friday. The post is temporary until 31st December 2014.

RELIEF BUS STATION RANGER

Relief Rangers are required on a zero hours contract basis to provide cover, as and when required, for regular staff based at Swansea City Bus Station.

Duties will include providing information, advice and assistance to customers and ensuring that a safe, clean, and pleasant environment is maintained inside the bus station. In addition you will monitor and control the vehicle movements on the external bus circulation area to ensure that these are carried out safely and in accordance with the Bus Station Operating Procedures.

Community Care Assistant - Long Term Service

We are seeking to appoint Community Care Assistants to join a well established Home Care Team, offering 25 working on a rota basis over 7 days, mornings and evenings, including weekends and bank holidays.

The successful candidates will have the attributes needed to undertake the range of duties associated with the post, including:

• Excellent communication both oral and written
• An ability to provide a high standard of personal care and support to people within their own home and to function as part of a team
• A commitment to support the authorities reablement agenda

The successful candidates will be offered to work in any one of the following service areas:

DOMICILIARY CARE ASSESSMENT SERVICE (DCAS)
A time limited service that aims to provide an assessment for adults living in the City and County of Swansea who are deemed as needing a package of Domiciliary Care. To promote independence, ensure people have packages of care tailor made to meet their individual needs and to enable as many people as possible to remain at home longer.

IN HOUSE HOME CARE SERVICE (LONG TERM SUPPORT)
The Long Term Service is a well established Service having been in place since the 1960’s. Although at it’s inception it was known as the Home Help service and was more about providing people with some Domestic support, it has over time developed more into what is known now as the Home Care Service. Our remit now concentrates more on the provision of personal Care and more about supporting people to remain at home whilst promoting independence.

RESIDENTIAL CARE ASSESSMENT SERVICE (RCAS): step up, step down is a provision that support Adults who have been deemed as requiring Long Term Residential Care, but wish to remain in their own home, but need a short period of care and support to make this a possibility. This can include people who are currently at home, but in need of extra support. This part of the service is linked to assessment beds and the team work in and out of the assessment unit enabling a robust assessment for individuals.

INTEGRATED GOWER CARE TEAM (IGCT) is a joint initiative between Swansea Council’s Social Services and the ABMU Health Board. Health & Social Care staff from both organisations work together to provide care and support to Gower residents.
The support will include:

• Re-ablement support for people who have experienced a period of ill health
• long-term domiciliary care (e.g. personal care)
• Continuing Health Care support
• end of life care
• a response service which provides care at home in an emergency.
These posts will provide practical help, support and reablement to a range of people including younger [over 18] and older people whose physical disability / illness means daily living tasks are a problem.

We will offer you a comprehensive induction programme and ongoing training. You will receive regular supervision and attend supportive team meetings. You will also be provided with a uniform and footwear.

The ability to travel within the County is essential. You will need to have access to your own transport and telephone.
