Residential Reablement Services

Most of us want to remain living at home, being as independent as possible. Our short stay Reablement Services offer specialist care and encouragement to return to independent living after a period of ill health.

In recent years Social Services has seen an increase in the number of people who are admitted into a care home after a crisis or period of illness. Whilst the initial plan is often to return home, this may not happen without the right kind of support. People can quickly lose their confidence and independence whilst in a care home and end up staying there long term.

Our Reablement Service at Bonymaen House supports older people who are experiencing a period of ill health at home, and need more support than normal while they are recovering.  It could also be suitable for someone who is in hospital and needs support to regain skills and confidence to be able to return to live independently at home.

This short-term service also gives us an opportunity to look at your longer term needs and what support you may need when you return home.

We also have a specialist Assessment Unit which offers a similar reablement service for people with dementia at Tŷ Waunarlwydd.

The two services provide all the professional care and support that is needed to maximise your independence from the minute the service starts, with the clear purpose of getting you home as soon as possible. There is no charge for your stay during your assessment period.  This could be anything from a few days up to a maximum of six weeks.  To use this service you must be referred by a social worker or health care professional.
